Scope of Services: Our home health service will provide medical and non-medical care to patients in their homes. We will offer services such as skilled nursing care, physical therapy, occupational therapy, speech therapy, home health aide services, and medical social work.
Patient Eligibility: Patients who are eligible for our home health services must have a physician’s order, be homebound, and require skilled care on an intermittent basis.
Payment: We accept payment from Medicare, Medicaid, private insurance, and self-pay. Payment for services is due at the time of service, and we reserve the right to bill the patient or their insurance company for any unpaid balances.
Patient Responsibilities: Patients are responsible for providing accurate information about their medical history and condition, following their care plan, keeping scheduled appointments, and reporting any changes in their condition to their healthcare provider.
Privacy and Confidentiality: We will protect the privacy and confidentiality of our patients’ medical information in accordance with HIPAA regulations.
Termination of Services: We reserve the right to terminate our services if a patient no longer meets the eligibility criteria, if the patient’s condition improves to the point where they no longer require our services, or if the patient fails to comply with their care plan.
Liability: We are not liable for any damages, injuries, or losses that occur as a result of a patient’s failure to follow their care plan, or as a result of any actions taken by the patient or their family members.
Complaints and Grievances: Patients have the right to file complaints and grievances with our agency, and we will work to resolve any issues in a timely and respectful manner.
Changes to Terms and Conditions: We reserve the right to make changes to our terms and conditions at any time, and will notify patients of any changes in writing.
